Tracking who modified specific fields (not records)

Does anyone know of a way to track "WHO" and WHEN" specific fields are
modified?

I need to know who added specific data to specific records even after other
parts of the records have been updated.

Any ideas?

A modification of a field, is a modification to a record.

You can track field modifications using one of several techniques available,
