Simple MIDI pass-through application

I want to be able to play my keyboard and be able to select which instrument
it will sound like on the PC. Since I already have it hooked up to my MIDI
input port this is fine.

Is there an application that will simply let you select which instrument the
MIDI input data should be played as, and also which MIDI output interface on
the PC it should use? At the moment I am using a very old version of
Midisoft and "Pass-Through" mode, but Midisoft has to be in focus for it to
pass through the MIDI signals. This makes it very difficult to play sheet
music which is displayed in another application.

I'm looking for a similar thing, except instead of running on a PC
it would be a pedal board with maybe half a dozen foot-buttons.
I should be able to program Button 3 = Patch 47 and so on.
Then I could play the keyboard and change sounds instantly.
Does anybody make anything like this ?
I wouldn't really want to deploy a whole CPU and OS to run the thing...
