Adding "New Page" to Document Menu in Acrobat

Many years ago, I read about a really neat trick whereby one can add 
"New Page" to the Document Menu in Acrobat 5, 6, and 7.  Now I've 
forgotten how to do it.

If anyone reading this post knows how, please help.

I imagine it was by using a file level JavaScript (i.e. a .js file
sitting in an Acrobat user directory that is parsed at startup) that
would add the item to the menu, and when clicks would run the
app.newDoc() JavaScript method.
