convert Word template (.dot) to editable pdf

Hi there,

I would like to convert a MS word template document (form to be filled
in including check boxes etc.) into a pdf document that could be
filled in, saved and sent back.

The reason for that is that I intend to send my template form to
people behind a firewall that might not let a *.dot document through.

Any suggestions how to solve that?

1. Make a Word document using the template
2. Make the Word document into a PDF
3. Using Acrobat Professional, recreate the form fields
4. Make sure all your end-users have Acrobat Standard or Acrobat
Professional (since the free Reader cannot save filled in forms).

Or
4. Implement a web based system where the form is submitted to a
script (CGI, ASP, etc.) and processed.

Pascal, this does not necessarily mean that the form data is submitted to a
remote location via the Internet; but rather, that a "web-like" solution is
employed. The required (e.g., CGI) server could be hosted on any of the
computers on the LAN behind the firewall. This would be a safer approach
than using an external web server, as access could be limited to only the
computers on the LAN behind the firewall.
