"Resetting scsi bus, got incorrect phase from"???

I've seen several people ask about this warning message in
/var/adm/messages, but have not really seen an answer out there... 
Any one know what it's griping about, and how to fix it?

This message indicates that target 0 lun 0 (the 0,0) got
out-of-phase with the rest of the scsi bus, so the controller
-- glm0, most likely your boot scsi controller -- decided to
reset the whole bus.

I'd look at the iostat -En output for this lun (probably sd0)
and check the error counts.

You probably (but not definitely) need to get it replaced in
short order.
